Our Social Prescribing team here at York CVS is part of the new Frailty Advice and Guidance Hub at Acomb Garth Community Centre. This is a fully integrated collaborative team working to support people who are vulnerable to stay safe and well in their homes.

We are pleased to announce that this service is now available to the VCSE sector Monday to Friday. This multi-disciplinary approach includes GP’s, Adult Social Care, Community Response Team, York Integrated Community Team, Social Prescribers and the voluntary, community and social enterprise (VCSE) sector, including our partners Age UK, and can provide support such as helping to get food, medications and increase in care during the acute phase.

In the first few weeks of this new service being in operation, the team has received referrals from Yorkshire Ambulance Service and GP’s and has prevented 29 unnecessary hospital admissions and supported 76 vulnerable people in need of help during a physical crisis.

When would we access this service? 

If a person you are working with needs urgent physical care, say they have fallen and they are injured, or you think they may be unwell you can call the numbers below. If you are unsure whether you need to call 999 or not, then this is the right service to call for advice.
