Are you passionate about supporting people living with dementia and their families to live well and remain independent for as long as possible?
York CVS is recruiting a Dementia Social Prescriber to join York’s Community Frailty Hub. Working as a fully integrated member of the multidisciplinary team, you will provide personalised support to people living with dementia and their unpaid carers, helping them identify what matters most, achieve their goals, and access the support, services, and community resources they need.
You will also work closely with health, dementia advisors, social care, and voluntary sector partners to reduce isolation, improve wellbeing, strengthen support networks, and enable people affected by dementia to continue living well within their communities.
This is an exciting opportunity to combine the strengths of social prescribing with an innovative, evidence-based approach to dementia support, delivering interventions that focus on independence, wellbeing, carer resilience, and quality of life.
We are looking for someone who:
• Has experience working with people living with dementia, older people, carers, or vulnerable adults.
• Is passionate about person-centred, strengths-based support.
• Has excellent communication and relationship-building skills.
• Can work confidently within a multidisciplinary team while maintaining strong links with community and voluntary sector organisations.
• Is committed to improving outcomes for people living with dementia and their families.
What you’ll be doing:
• Supporting people living with dementia and their carers to identify goals and priorities.
• Providing personalised social prescribing support and wellbeing planning.
• Connecting people to local community resources, services, activities, and support networks.
• Working alongside clinicians and community partners within the Community Frailty Hub.
• Helping people maintain independence, confidence, and quality of life.
• Contributing to service evaluation and continuous improvement.
