If you are highly organised, can work in a fast-paced environment we would like to hear from you. As Co-ordinator you will strive to support people when they need us most, this can be after a crisis, short illness or even facing a very different future after diagnosis.
You will receive referrals, from health professionals, family members, and service users. The Co-ordinator will assess the needs of service users and risks and then prioritise needs, and allocate to team members, accordingly, recording each referral on Charity log accurately and in a timely manner. Ensuring smooth running of service daily with support of the manager, supporting and co-ordinating the team as needed.
You will be working as part of a small team to provide the best support we can to give the service users the best service we can. We are a patient led service; they are their goals, health and independence come first.
