Social Prescriber - Transgender and Non-binary
Salary: £29,768 | Hours: 15 per week | Fixed Term: 12 month fixed term contract with the possibility of an extension

York CVS are looking to recruit a passionate and empathetic individual to work on a new project supporting individuals who are part of the Transgender and Non-Binary community including those wider identities such as gender-diverse and gender questioning.
Transgender and non-binary (TGNB) individuals experience considerable barriers in accessing healthcare and support services. These individuals often face discrimination, long waiting times for gender-affirming care, and a lack of inclusive services.
The Proactive Social Prescriber will offer holistic, non-clinical support tailored to the needs of Transgender and Non-Binary individuals. The service focuses on improving wellbeing and addressing wider determinants of health, such as:
- Access to safe and inclusive housing, employment, or education
- Access to financial support and benefits advice
- Social connection and peer support
- Navigating discrimination or barriers in public service
You will proactively work with individuals, getting to know the person, developing a positive relationship with them, understanding the challenges they face and the changes they want to make. You will work alongside patients to create personalised care support plans facilitating empowerment and independence.
Social Prescriber – Respiratory - COPD
Salary: £29,768 | Hours: 37 per week | Fixed Term: 12 month fixed term contract

We are looking to recruit a passionate, compassionate, and highly motivated Social Prescriber to join our Proactive Social Prescribing (PSP) Respiratory Project, supporting individuals living with COPD to overcome barriers to accessing healthcare and improve their quality of life.
Since launching in 2024, this project has delivered positive outcomes, including reduced hospital admissions and improved wellbeing for patients. This is an exciting opportunity to be part of a successful, growing initiative and make a meaningful difference every day.
The role focuses on providing holistic, person-centred support, addressing wider determinants of health such as housing, financial wellbeing, social isolation, and access to services. You will proactively engage individuals, build strong relationships, and empower them to take control of their health and wellbeing.
This role requires excellent relationship-building, organisational, and partnership-working skills. The successful candidate will manage a caseload, contribute to service development, and work collaboratively across health and community systems.
Social Prescriber - Dementia Programme

In this rewarding role, you’ll deliver the GRACE (Goal-Directed Accessible & Evidence-Based Care) programme – an innovative, research-backed approach designed to help people living with dementia, and their carers, live well at home.

This is more than just a job. It’s an opportunity to support people facing real-life challenges and help them take meaningful steps back into work, education, and community life.
About the programme
The Trailblazer Programme is designed to tackle economic inactivity and support individuals aged 16–64 who are not currently in education, employment, or training (NEET).
You’ll be part of a forward-thinking approach that brings together health, employment, and skills support into one joined-up, person-centred service, helping individuals overcome barriers and move forward with confidence.
What you’ll be doing
In this varied and rewarding role, you will:
- Provide one-to-one, holistic support to individuals, helping them access pathways into work, education, or training
- Work collaboratively within a multi-disciplinary team alongside GPs, Case Managers, and Nimbuscare staff
- Lead on case management, safeguarding, and complex care coordination
- Build strong relationships with local authorities, VCSE organisations, and community services
- Help shape and develop the service through data collection, reporting, and continuous improvement
- Identify gaps in support and work creatively with partners to bridge those gaps
- Connect with a wider network of 26 Trailblazer projects across York & North Yorkshire
- Contribute to reducing health inequalities and improving access to services
