Community Support Worker

The Community Support Worker is a rewarding role for people who thrive on supporting people to retain or regain their independence and remain safe in their local community.

As a Community Support Worker you will:

Provide initial support to the older person for up to 16 weeks. If the person requires further support, they will be supported to access a range of other services.
Carry out a wide range of activities to enable the client to use their own strengths and resources to remain at home, prevent hospital admission and engage in their community.
Visit clients usually in their home to identify what needs they have to enable them to maintain their independence and to help address these needs where appropriate through regular contact.
Create befriending matches when needed, choosing the right people for the person you are supporting.
Providing support and training to befriending volunteers.
