We’re recruiting for a Social Prescriber Manager (Development and Proactive) to take on an important dual role within our city-wide Social Prescribing Service.

About the role
You’ll lead the operational management and continued development of our Proactive Social Prescribing Service, which currently includes our Respiratory (COPD) and Trans and Non-Binary projects.
Alongside this, you’ll work across our wider Social Prescribing Service to strengthen workforce development through:
- Coaching and mentoring
- Professional development
- Supporting colleagues to develop their skills and practice
- Service improvement
- Sharing learning and good practice
Working closely with our Senior Project Manager and other Social Prescribing Managers, you’ll help ensure our services remain safe, effective, person-centred and responsive to the changing needs of individuals and communities across York.
This is an opportunity to make a real contribution to the development of Social Prescribing across the city, while supporting a skilled and committed workforce to deliver high-quality support.

We’re looking for someone with strong management experience who can support and develop others while also having an understanding of person-centred services and the wider factors that influence people’s health and wellbeing.
You’ll need to be confident working collaboratively, supporting service improvement and responding to the changing needs of individuals and communities.
We’re particularly interested in people who share our commitment to reducing health inequalities and improving access to support.
