York CVS is looking for a passionate and inspiring Social Prescriber to join our dynamic Social Prescribing Team. In this rewarding role, you’ll deliver the GRACE (Goal-Directed Accessible & Evidence-Based Care) programme—an innovative, research-backed approach designed to help people living with dementia, and their carers, live well at home.

You’ll receive full GRACE facilitator training and work in partnership with Queen Mary University of London, contributing directly to national dementia research that is improving lives across the UK.

Key responsibilities:
- Deliver eight personalised, one-to-one sessions to each family
- Empower carers and individuals to set meaningful, achievable SMART goals
- Help families understand their support networks, identify gaps, and access vital services
- Guide participants through GRACE modules (wellbeing, communication, mobility, safety, routines, behaviour support, and more)
- Build trusted, supportive relationships while maintaining professional boundaries
- Provide holistic, person-centred support throughout the programme
- Work collaboratively within a multi-disciplinary team, including GPs and Nimbuscare staff
- Lead on case management, safeguarding, and complex care coordination
- Build strong partnerships with VCSE organisations, local authorities, and community services
- Contribute to service development, quality improvement, and impact measurement
- Help reduce health inequalities and improve access to support services
- Identify gaps in provision and collaborate with partners to address them

Fixed Term: 12 month fixed term contract
An enhanced DBS check is required

Location: Working in the local community, may be a variety of locations, including visiting people in their own homes. Office bases - York CVS & Nimbuscare, Gateway 1
