The Humber and North Yorkshire ICS (Integrated Care System) is a partnership of organisations who are working together to develop a three-year plan for dementia. This three-year plan aims to improve experience and outcomes at all stages of the dementia journey from first recognition of symptoms, right through to end of life care.

The Humber and North Yorkshire dementia programme including the Alzheimer’s Society would welcome the opportunity to listen to the views of our local communities to make sure that our three-year plan is based on lived experience and reflecting the needs of our population so in addition to the four public consultation events that are been held in the region, there is also the option to complete the attached questionnaire.
