Social Prescriber – Transgender and Non-binary

York CVS are looking to recruit a passionate and empathetic individual to work on a new project supporting individuals who are part of the Transgender and Non-Binary community including those wider identities such as gender-diverse and gender questioning.

Transgender and non-binary (TGNB) individuals experience considerable barriers in accessing healthcare and support services. These individuals often face discrimination, long waiting times for gender-affirming care, and a lack of inclusive services.

The Proactive Social Prescriber will offer holistic, non-clinical support tailored to the needs of Transgender and Non-Binary individuals. The service focuses on improving wellbeing and addressing wider determinants of health, such as:
· Access to safe and inclusive housing, employment, or education
· Access to financial support and benefits advice
· Social connection and peer support
· Navigating discrimination or barriers in public service

You will proactively work with individuals, getting to know the person, developing a positive relationship with them, understanding the challenges they face and the changes they want to make. You will work alongside patients to create personalised care support plans facilitating empowerment and independence.

Key Responsibilities:
· Build warm, empowering relationships while maintaining professional boundaries.
· Provide one – one holistic support to people who are enrolled onto the project
· Create person centered care support plans
· Attend and contribute to MDTs when appropriate
· Lead on case management, safeguarding and complex care coordination
· Build and maintain strong partnerships with VCSE organisations, local authorities and community services
· Contribute to service development, quality improvement, and outcome measurement
· Support system-wide work to reduce health inequalities and improve access to services
· Identify gaps in provision and work with partner organisations to bridge the gaps.

About You:
· Experience in social prescribing, community development, or complex needs services
· Strong understanding of the VCSE sector and community-based support systems
· Experience of working with people facing multiple disadvantages (e.g. mental health, long-term conditions, social isolation, homelessness, substance misuse)
· Excellent communication, relationship-building and partnership-working skills
· A strong commitment to inclusion, equity, and person-centred practice
· Ability to work autonomously while contributing effectively to multidisciplinary teams
· An understanding of the LGBTQ+ community and inclusive language

The successful candidate will be required to complete an enhanced DBS check.
Fixed Term: 12 month fixed term contract with the possibility of an extension
