UPDATE: York VCSE Assembly - NHS Humber and North Yorkshire ICB - Organisational Change Update

 

Dear colleagues and partners

Thank you for your continued engagement and support as we have worked through the consultation phase of our organisational change programme.

As you may recall, on 19 November 2025 we launched formal consultation with staff on a proposed new operating model and structure for NHS Humber and North Yorkshire ICB, alongside proposals relating to future ways of working. The consultation closed on 9 January 2026 and the outcomes were shared with staff last week.

Following consideration of the feedback received and further national guidance issued during the consultation period, the Board has now agreed the final operating model, which we intend to implement from 1 April 2026. We have attached a slide deck which covers the points below in greater detail.

 

Our future operating model 

The final model reinforces our role as a strategic commissioner, operating within a significantly reduced running cost envelope while strengthening clarity of accountability and delivery.

 

Four business units

We will have four business units which will work together to design, commission, procure, evaluate and pay for impactful healthcare services delivered through two pillars across five commissioned programmes of care.

  • Insight and Commissioning Strategy: The intelligence engine and strategic brain – sets the direction and determines the response
  • Clinical Leadership: The clinical advisor – ensures safety, quality and health protection
  • Purchasing and Outcomes-based Contracting: The financial architecture and contracting engine –procures and secures delivery
  • People and Organisational Effectiveness:  The connector, communicator and organisational backbone

 

Five interconnected Programmes of Care

Our Programmes of Care will lead system-wide collaboration and commissioning design across:

  • Family Health
  • Neighbourhood Health
  • Mental Health, Learning Disability and Neurodiversity
  • Planned Care, Cancer and Medical Specialties
  • Urgent and Emergency Care
We will continue to commission once for Humber and North Yorkshire where appropriate, while ensuring the model allows for place-based nuance where this is essential.

 

Two delivery pillars

Commissioning and performance activity will be organised through two sector-aligned pillars:

  • Neighbourhood Health and Primary Care
  • Acute and Mental Health
These pillars bring together commissioning design, contracting, performance oversight and provider relationship management, ensuring stronger alignment between commissioning intent and delivery.

 

Strengthened place-aligned partnership working

In response to feedback, we have strengthened our place interface through the creation of three senior Directors of Commissioning: Partnerships and Engagement, aligned to:

  • York and North Yorkshire
  • North Lincolnshire and North East Lincolnshire
  • Hull and East Riding
These roles will provide clear senior leadership at place level and act as key points of contact for local authority leaders and partners. Each place will also have aligned commissioning capacity, including a strengthened focus on children and young people and neighbourhood health planning.

We remain fully committed to strong partnership working across all Places and to ensuring that local priorities shape commissioning decisions.

 

Future ways of working

Alongside the operating model, we have confirmed future contractual base arrangements.

Posts will be designated as Agile, Community-Based or Fixed. Colleagues in Agile or Community-Based roles will be able to select either Health House in Willerby or home as their contractual base.

While this means we will no longer maintain a fixed office footprint aligned to each Place, we remain committed to purposeful presence within communities. We will continue discussions with partners regarding access to touchdown spaces and shared working environments to support collaborative working across Humber and North Yorkshire.

 

What this means for partners

For our system partners, the new model gives clearer senior contacts at place level and stronger alignment between commissioning strategy and delivery. It ensures a consistent population health perspective across programmes, maintains focus on neighbourhood health and integration, and supports robust governance arrangements. Our priorities remain the same: maintaining stability, commissioning safe, high-quality services, and working collaboratively with partners throughout the transition.

 

Next steps

We plan to implement the new structure from 1 April 2026, once appointments are confirmed. We will keep partners informed as implementation progresses, particularly on governance, commissioning interfaces, or joint working arrangements.

York VCSE Assembly 

The York VCSE Assembly – Health and Care is open to all Voluntary, Community and Social Enterprise (VCSE) organisations in York. We meet on a bi-monthly basis to focus on health and care related topics. 

The Assembly acts as a mechanism to connect York’s VCSE sector with Humber and North Yorkshire Health and Care Partnership, our area’s Integrated Care System, the Humber and North Yorkshire VCSE Collaborative and the York Health and Care Partnership.

By joining the York VCSE Assembly – Health and Care, and sharing your knowledge and areas of expertise, you’re supporting us in positioning the VCSE sector as a key partner in the planning and delivery of health and care across our city.

Dianne Willcocks, Vice Chair, York Older People’s Assembly (YOPA) provided an update regarding the York 50+ Festival which will take place during 20 September – 1 October. There will be a Day of Celebration and a central parade in York on Saturday 27 September from 1pm. Dianne is keen to include as many VCSE organisations as possible, so please contact her directly via the details below.

VCSE Workforce Mapping Survey 
During the meeting we presented a piece of essential research which is currently running until the 28 February. We’re asking all York VCSE organisations to complete this survey to map our sector’s workforce. The data provided will support future investement into the sector.

2024 Meeting notes

Representation

Climate Change, Health and the Role of the VCSE Sector - 10 October 2024

This event was held as part of York Climate Weeks and focused on the impacts of climate change on health and health inequalities, and the role of the Voluntary, Community and Social Enterprise (VCSE) sector. We welcomed Peter Rodderick, Director of Public Health, City of York Council, who introduced the session and the York Climate Commission, and Jess Marshall, Public Health Manager – Health Protection who spoke about the connection between climate change and health.

This Assembly meeting focused on ‘Health and Care at Place (York)’. We welcomed Sarah Coltman-Lovell, the new Place Director for York and we explored how the Health and Care Partnership in York works, and how as a Sector we can connect with the Partnership.

York VCSE Assembly meetings going forward 
At the end of the meeting, we touched upon how we propose to take the York VCSE Assembly meetings forward. We proposed holding one larger central meeting per quarter with one more focused and topic specific meeting in between. We also proposed reviewing our Terms of Reference at the next meeting to make sure it is still relevant. 

York VCSE Assembly meeting - 25 January 2023

This Assembly meeting focused on ‘Volunteering at the heart of the health and care system’, which is a workstream within the ‘180 days of workforce plan’ (Humber and North Yorkshire Health and Care Partnership). We explored and discussed what we thought were appropriate roles and what were not. 

During the meeting we asked atendees to review the workstream’s Volunteering Principles and Charter document – we will share this information as soon as the document has been finalised and the feedback points have been captured.

York VCSE Assembly meeting - 21 June 2022

The Assembly are keen to start getting some project/work ideas together so we’re ready for potential investment opportunities coming from the ICS/York Alliance/other sources – with a specific focus on health inequalities and workforce. Ideally, these will be drawn up collaboratively between members of the Assembly. We’ll (York CVS) be developing a template for ideas and pulling together the key challenges we have in York to give some food for thought. This template will be shared with attendees.
